Q:   Does your office accept my insurance?

A:   Each provider is in private practice and selects their own insurance contracts.  The office staff can help determine if your insurance is in-network or out-of-network and any applicable benefits.

Q:   How do I become a new client? 

There are (2) options to start the new client process:

A1:   Register online - complete the online registration for the therapist of your choice.  After the online registration is complete, the office staff will call you within a week to review insurance benefits (if applicable), costs and schedule availability.

 

A2:  Call the office at 574-534-2161 ext 0.  The office staff will take your new client information over the phone.  We will call you back within a week to review insurance benefits (if applicable), costs and schedule availability.
 

Q:   Do I need a referral from my doctor to schedule an appointment if I am a new client?

 

A:   While our office does not require a referral to schedule an appointment with a therapist, we gladly accept doctor referrals. Doctors may fax demographic, insurance, and  referral information to (574) 534-3887. New clients should call the office at (574) 534-2161 ext 0. to schedule an appointment with a therapist or register online.
